This section includes the forms and other documents utilized by Kentucky Office of Vocational Rehabilitation counselors to provide services to Kentuckians with disabilities.
Program Policy and Support
The Program Policy and Support Branch provides support to staff and the agency as a whole, including tasks such as CMS change requests, scanning and archiving, forms, data requests, and ensuring agency compliance with federal and state requirements.

Earlier this year, the University of Kentucky (UK) - Human Development Institute's (HDI) Evaluation Unit was contracted by OVR to develop a 2024 KY OVR Comprehensive Statewide Needs Assessment (CSNA) through gathering information and analyzing current and projected needs of individuals with disabilities within Kentucky. Now, the team through HDI is requesting your help.

The OVR Counselor and Staff Survey Link (opens link) was created by HDI to gather feedback from all OVR Staff and Counselors. The survey is confidential, and steps will be taken to keep your information anonymous. The survey is scheduled to remain active until January 5, 2024, but the team would greatly appreciate any and all early survey completions.
