Open Records Requests

OVR may accept records requests from any resident of the Commonwealth as defined in state statutes. Telephonic records requests may be accepted only from the individual requesting the individual’s own records or from an authorized representative already on file requesting the individual’s records.

Any third-party records requests shall be submitted in writing using the standardized form developed under KRS 61.876(4) titled Request to Inspect Public Records (Form OAG-01, June 2021).      

Requests can be sent to OVRRecords@ky.gov​ for processing. You may also refer to the Education and Labor Cabinet's website​ for further details regarding open records requests.
